When Can I Join Hospice?

You may enter a program if your doctor states that you have a terminal illness and the death can be expected in 6 months or less .You can stay in hospice beyond that time if your doctor and team decide you still have only a short time to live. Hospice isn’t always a permanent choice. For example, if your kidneys are failing, you might choose the hospice program rather the continuing with dialysis. But you can still change your mind, stop hospice care and start back on treatments.

Other people may get better unexpectedly and quit the service with the option of returning later . Hospice differs from pallivative care, which serves anyone who is seriously ill, not just those who are dying and no longer seeking a cure .

Who Pays For The Hospice?

In most states, MEDICARE and /or MEDICAID provides hospice coverage as well as most private insurance plans. Coverage includes everything involved in hospice care for Nurse & CNA visits, durable medical equipment, therapy services as appropriate, and all medications related to terminal diagnosis.

No matter the location, hospice care is centered on comfort, dignity, and quality of life, with an emphasis on personalized support for both the patient and their family.

Levels Of Hospice Care

Routine home Care

Ongoing end - of - life care provided at home or in a nursing home

Continuous Care

Short - term end - of - life care provided by nurses and hospice aids to patient in crisis to keep the patient at home as determined by physician

Respite Care

Short - term stay ( up to 5 days ) in a nursing home or hospital

General Inpatient Care

Temporary intensive care at a hospital or nursing home to manage pain or symptoms that are out of control as determined by a physician

Medical Director

Certifies the terminal prognosis with the attending physician, leads the interdisciplinary team in developing a plan of care and supports the entire hospics team

Registerd Nurse

Coordinates a plan of care for every patient, provides patient car to manage symptoms and medications. The nurse is the liaison between patient, family and physician

Hospoce Aide

Provides persinal care to the patient, reports any change to the RN Case Manager

Social Workers

Assesses the patients emptional , social and financial need, provides direct counseling and bereavement support to make referrals to approriate community egencies

Chaplain

Assesses spiritual needs and spiritual counseling utilizes the services of local religious congregations and communities to fulfill the needs to each individual

Dietary / Nutrtion

A specialized dietary regiment creates for the patient and eductes the family during the implementation process

Volunteers

Provide companionship and support to both the patient and their loved ones

Bereavement

Helps to plan memorial services and develop a plan of care for the family as part of the thirteen month bereavement process . Also able to provide grief support and make referrals to grief counselors
